c90c93afd6 Add comprehensive Self-Learning System documentation
This guide explains:
- How the real-time learning system works
- The 10 patterns Claude detects (switching, decided_to_use, bug_fixed, etc)
- How it's now auto-activated in initialize-project
- Real-world examples and use cases
- Troubleshooting and advanced topics

Key learning: The system is designed to make Claude a genuine team member
who learns from conversations and remembers important project decisions.

Now fully integrated into the project initialization workflow.

f63cc7e49d Fix RQ worker startup command in start_redis_workers.sh
Changed from broken 'python3 -m rq worker' to proper 'rq worker' CLI command.
The python -m invocation was failing with 'No module named rq.__main__'.

The RQ workers are now fully operational and listening on all three queues:
- claude-os:learning
- claude-os:prompts
- claude-os:ingest

e64e4fc495 Improve fallback answer generation - create real answers from documents
Replace generic 'Retrieved N documents' message with actual answer synthesis
from the top 3 documents when LLM synthesis fails. Extracts:
- Document previews (first 300 chars)
- Document filenames from metadata
- Creates readable answer format

This ensures users get useful information even when LLM synthesis encounters errors.

11d47c4067 Add fallback error handling for LLM synthesis with TextNode attribute issues
Wrap synthesizer.synthesize() in try-catch to handle AttributeError gracefully
and fall back to returning raw documents when LLM synthesis fails.

This allows the chat feature to work even when there are Pydantic or llama-index
compatibility issues with TextNode attributes.

0d8d6d7bb8 Disable SimilarityPostprocessor to avoid Pydantic score validation errors
The SimilarityPostprocessor tries to access node.score which isn't supported
in the current llama-index version and causes Pydantic validation errors.

Since we're already getting sorted results from vector similarity search,
disable the postprocessor and use retrieved nodes directly.

This fixes chat and search endpoints.

5a14284d62 Fix TextNode score handling - store in metadata instead
TextNode doesn't support a direct score attribute in this version of llama-index.
Instead, store the similarity score in the node's metadata during creation and
retrieve it from metadata during response formatting.

This ensures full compatibility with Pydantic validation and prevents attribute errors.

2a0f07f951 Fix TextNode score validation - pass score during initialization
Instead of setting node.score after creation (which fails Pydantic validation),
pass the score parameter during TextNode initialization. This ensures the score
field is properly validated during object creation.

This fixes the error: 'Object has no attribute score' in chat and search endpoints.

c1c8dc4da0 Fix Pydantic TextNode validation error - convert numpy.float32 to Python float
The similarity score was being set as a numpy.float32 from SQLite distance calculations.
Pydantic expects Python native float types for the score field, causing validation errors
when querying MCPs or using chat features.

Fix: Convert similarity score to Python float using float() conversion.

bb3f468dd4 Add setup.sh and fix database path consistency across services
Add comprehensive one-command setup.sh script that handles:
- Prerequisites validation (Python 3, Git)
- Virtual environment creation and dependency installation
- Ollama installation and model download (llama3.1, nomic-embed-text)
- Redis installation and startup
- Directory and database initialization
- Frontend npm dependencies
- Clear progress output and next steps

Fix critical database path issues:
- Use absolute path calculation in config.py to ensure consistency
- Export SQLITE_DB_PATH environment variable in startup scripts
- Prevents database separation between app and MCP server

Fix document metadata field naming:
- Change metadata field from 'source' to 'filename' in MCP ingest
- Ensures consistency with existing document format
- Fixes document visibility issue in API endpoints

Update startup scripts to support environment-based database configuration.

e6400b5b4a Add Memory MCP Guide - persistent context across sessions
Memory MCP solves a critical problem: context loss between sessions.
Instead of searching through knowledge bases, Memory MCP provides:

✓ Instant automatic recall (no search latency)
✓ Natural context reference in conversations
✓ Zero setup required (just say 'Remember:')
✓ Works even when embedding service is down
✓ Full-text searchable JSON storage
✓ Human-readable memory format

Key Features:
- Say 'Remember: [info]' to save
- Ask 'What did we work on yesterday?' for automatic recall
- Search memories by title, content, or tags
- All memories stored in ~/.claude/mcp-servers/memory/data/memories.json

Use Cases:
- Project context across days/weeks
- Technical insights from debugging
- Architecture decisions and rationale
- Quick reference facts about the codebase
- Avoiding repetition in explanations

Memory MCP + Code-Forge KB + Git Hooks = Complete knowledge system
- Memory: What we're working on this week
- KB: How the entire codebase works
- Hooks: Automatic updates on commits

This is how professional AI coding tools maintain context.
