Add safe mode enforcement and redesign recovery dialog
...
Safe mode now loads only preloads with '00_' prefix or legacy sentinel.
Recovery/dev dialog is replaced with a dark-themed UI featuring safe
mode toggle, custom URL loader, and improved confirmation modals.
